Case Management

A Case Manager guides and supports both singles and families, of all ages, making a positive impact while assisting with life's challenges. The Case Manager assists households in getting life's basic necessities, as well as some important extras such as back-to-school supplies, Easter baskets, and Christmas gifts. Is there a birthday coming up?  How about a "birthday bag" for the kids?  We supply a bag filled with a cake mix, frosting, drink mix, party plates, napkins, candles and more, because every child should feel special on their birthday.  For those moving into their senior years, we understand that those needs can differ from younger people.  Our Case Managers are knowledgeable about the concerns and problems you may face and many times we are able to meet those needs in-house.  When we can't, we make referrals to other agencies yet stay connected as an advocate.  A Case Manager can also assist households who qualify for Low Income Home Energy Assistance Program (LIHEAP). This is an income-eligible program that begins in November of each year to assist households with utility bills (electric or natural gas), or fuel bills (oil, coal, kerosene, wood, propane or bottled gas). If eligible, funds will be sent directly to the utility/fuel dealer.  If you find yourself in need, fill out the form below and we will do our best to help!
