Provides transportation to and from medical appointments. Transportation is provided by volunteers who donate their time and vehicle to transport individuals to medical appointments. Transportation is for those who are unable to drive and have no one who can provide transportation.

Every attempt will be made to accommodate a request, but transportation is dependent entirely upon availability of a volunteer. If we are unable to provide a volunteer, notification will be given one day prior to the scheduled appointment. |

| Q. Who is eligible for transport? |

Residents of the area served by Boyertown Area Multi-Service who are unable to drive and have no one who can provide transportation are eligible. The individual needing transport must be ambulatory and able to get to and from the vehicle without assistance. |

| Q. How do I make a request for transportation? |

All requests need to be made at least one week in advance of the appointment. If it is an initial request for transportation, it will be necessary to speak with a case manager so that an intake form can be completed and guidelines reviewed. |

| Q. What types of information are required? |

You will need to provide:
- your name, address, and phone number.
- a copy of a medical assistance card (Access card) or Department of Public Assistance funded insurance card, if applicable.
- the name, address, and phone number of the doctor or medical facility to which you are to be transported.
- the date and time of your appointment.

| Q. Can I take my child with me? |

Transportation is for the patient. Children are transported only if they are the patient. The child must be transported in proper restraints according to state law. No child will ride in the front seat. A child must be accompanied by a parent/guardian, as volunteers are not permitted to baby sit. |

| Q. Can the volunteer run errands? |

The volunteer is to provide transportation to the appointment only. Extra stops for medication are permitted only if arrangements are made with the transportation coordinator in advance. |

| Q. When is transportation available? |

Transportation is available Monday through Friday 8:30 A.M. to 4:30 P.M. No transportation is available evenings and weekends. |

| Q. How much does transportation cost? |
A. |
There is no set fee for transportation. Donations to Multi-Service to off-set any costs of transportation are welcome. |

| Q. Is transportation available for emergencies? |

No. Please call an ambulance for emergency transports. |

| Q. What if I need to cancel an appointment? |
A. |
Please contact the transportation coordinator as soon as possible if your plans change so that the volunteer does not make an unnecessary trip. Failure to notify Multi-Service about changes may result in the inability to transport in the future. |
